Indeed, you need an MT to move to brussels in order to live in a region where there is a hospital, and this is only Brussels. Normally you can get one for free from the government requesting it at citizen.info, but apparently someone already sent it to you.

Secondly, you can fight barehanded in the war, you don't need guns. Guns are only usefull to do more damage, but they cost a lot of money (you can buy them from the marketplace). You will be able to buy them later on in the game.
